Appointment Information

New patients- Please arrive 30 minutes prior to your appointment to fill out your New patient paperwork. you have the option to fill out forms on our patient portal. please ask our office staff for login info.

Established patients Please arrive 15 minutes prior to your appointment.

All Co-pays/deductible portions will need to be collected at time of service.

PLEASE TAKE YOUR USUAL MORNING MEDICATIONS WITH A SMALL SIP OF WATER BEFORE ARRIVAL AT THE CLINIC

1. Be sure to inform the doctor  if any of the following apply to you:
         ● You are taking a blood thinner, such as Plavix (Clopidogrel), Coumadin (Warfarin) Xarelto (Rivaroxaban), Eliquis (Apixaban) and Effient                           (Prasugrel).  BLOOD THINNERS NEED TO BE STOPPED 7 DAYS PRIOR TO PROCEDURE, PERHAPS LESS. Please discuss with the physician               for details.  
         ● You have fever or are taking an antibiotic
         ● You have an allergy to X-Ray dye
         ● You may be pregnant
2. Do not eat or drink 8 hours before the procedure if you will be having sedation.
3. Please arrange for someone to be present in the office during your procedure and drive you home. Your procedure may be cancelled if you        do not have a ride!!!
4. Wear loose fitting clothing. Sweat pants work well. Jeans are not allowed. If you are going to have procedure performed on your upper back      or neck, wear a shirt that can be easily removed.
5. Arrive 30 min before your scheduled procedure time.
6. An intravenous (IV) catheter may be placed in your hand or arm. This is for your safety and may be used to administer medications.
